We know referring offices need a process that is clear, fast, and easy to use. Advanced Retina accepts referrals by phone, fax, and referral form. Please include the patient’s contact information, insurance details, reason for referral, relevant exam findings, imaging if available, and the preferred location if known

You may refer a patient by submitting our referral form, faxing records, or calling our office directly. For urgent retina concerns, please call our office so our team can help route the patient appropriately.
